Behavioral Health, Chest Pain, & Chronic Disease Nurse Case Manager I
Location: 100% REMOTE, must hold Compact RN license
The Behavioral Health Case Manager I supports our members with serious substance abuse, substance abuse-related disorders, and/or mental health-related illness and is responsible for performing case management, telephonically, within the scope of licensure for these members.
Primary responsibilities include, but are not limited to:
Use appropriate screening criteria, knowledge, and clinical judgment to assess member needs.
Supporting Commercial Behaviorla Health Case Managent Nationwide
Conduct assessments to identify individual needs and develops care plan to address objectives and goals as identified during assessment.
Monitor and evaluate the effectiveness of the care plan and modifies the plan as needed.
Support member access to appropriate quality and cost-effective care.
Coordinate with internal and external resources to meet identified needs of the members and collaborates with providers.
Requires MA/MS in social work, counseling, or a related behavioral health field or a degree in nursing, and minimum of 3 years clinical experience in social work counseling with broad range of experience with complex psychiatric and substance abuse or substance abuse disorder treatment; or any combination of education and experience which would provide an equivalent background.
Requires a current, active, and unrestricted RN Compact license from the state in which you reside.
Telephonic and/or face-to-face behavioral health case management experience is required
- Experience and or training wtih Chest Pain and Chronic Diseases strongly preferred.
Applicable to Colorado Applicants Only Annual Salary Range*: $58.320-$72,900 Actual compensation may vary from posting based on geographic location, work experience, education and/or skill level. * The hourly or salary range is the range Anthem in good faith believes is the range of possible compensation for this role at the time of this posting. The Company may ultimately pay more or less than the posted range. This range is only applicable for jobs to be performed in Colorado. This range may be modified in the future.
