BH Case Manager Location: This is a remote work at home position that will serve our members in Kentucky. Following onboarding and training, local travel may be required. Hours: Monday – Friday 8am – 5pm
The Behavioral Health Case Manager is responsible for performing case management telephonically and/or by field visits for members with behavioral health and substance use disorder needs. This position is primarily member facing and involves a complete clinical assessment, a care plan and identifying resources within the community. Primary responsibilities include, but are not limited to: • Use appropriate screening criteria, knowledge, and clinical judgment to assess member needs. • Conduct assessments to identify individual needs and develops a care plan to address objectives and goals as identified during assessment. • Monitor and evaluate the effectiveness of care plan and modifies plan as needed. • Support member access to appropriate quality and cost-effective care. • Coordinate with internal and external resources to meet identified needs of the members and collaborates with providers.

Requires 3+ years of clinical experience in social work, counseling with a broad range of experience with complex psychiatric and substance abuse, or substance abuse disorder treatment; or any combination of education and experience, which would provide an equivalent background. Kentucky residency is required Travel is required visiting members a care facility.
Preferred Qualifications • Preferred knowledge of Kentucky Medicaid • Preferred Case Management experience telephonic and/or field-based • Preferred managed care experience • Experience in health coaching and motivational interviewing techniques preferred. • Experience with community mental health resources, CSB (Community Service Board), or similar is preferred. • Experience working in a field-based position. • Experience working at home in a virtual environment • Certified Case Manager preferred o Computer skills: You must have experience working with MS Office Products. You will receive training on Anthem-specific applications, however, you must be comfortable working on a laptop with web-based applications. If you are not computer savvy this may hinder your ability to be successful in this position.
