Behavioral Health Case Mgr II Location: Las Vegas, NV (field-based role)
Join a dynamic team of field-based case managers focused on working with members to improve their physical and behavioral healthcare needs. This position is field-based located in the Las Vegas, NV area. Up to 75% of local travel is anticipated.
Note: Field work is temporarily on hold due to the COVID 19 pandemic. Current work is telephonic based with an anticipated return to the field in the near future.
Responsible for performing case management duties within the scope of licensure for members with behavioral health, physical health and/or substance use disorder needs. Individuals on your caseload may be struggling with social determinants of health (i.e. homelessness, poverty, food insecurity, etc.). Primary duties may include but are not limited to:
Face-to-face visits with members in their home, hospital, and other community settings, such as doctor's offices, treatment centers, etc. (Temporarily outreach is telephonic only due to the pandemic.)
Use appropriate screening criteria knowledge and clinical judgment to assess member needs then develop care plan to address objectives and goals as identified during assessment.
Monitor and evaluate the effectiveness of care plans and modify as needed. Support member access to appropriate quality and cost effective care.
Coordinates with internal and external resources to meet identified needs and collaborates with providers.
Identify members at high risk for complications and coordinate care in conjunction with the member and the health care team.
Assists in problem-solving with providers and the health care team.
Become knowledgeable about multiple aspects of health insurance including Medicaid and Commercial health plans.
Participate in cross-functional teams projects and initiatives.
Must be located in the Las Vegas, NV area and have the ability to travel (temporarily telephonic only due to the pandemic) up to 75%.

MA/MS in social work counseling or a related behavioral health field or a degree in nursing, and minimum of 3 years of clinical experience in social work counseling with broad range of experience with complex psychiatric and substance abuse or substance abuse disorder treatment; or any combination of education and experience which would provide an equivalent background.
Current active unrestricted license such as RN LCSW LMHC LICSW LPC (as allowed by applicable state laws) LMFT LMSW (as allowed by applicable state laws) or Clinical Psychologist to practice as a health professional within the scope of licensure in applicable states or territory of the United States required.
Previous experience in case management and telephonic and/or in person coaching with members with a broad range of complex psychiatric/substance abuse and/or medical disorders. Managed care experience required.
For Government business only LAPC LAMFT (as allowed by applicable state laws) is also acceptable in addition to other licensure referenced above; and any other state or federal requirements that may apply.
Experience in health coaching and motivational interviewing techniques preferred.
Members served often have health conditions which require specialty providers. The ideal candidate will have the ability to assist members who have Anthem Commercial insurance with a variety of health conditions.
