This position is remote.

Operating hours: Monday - Friday 7 a.m. - 7 p.m. CST.

Responsible for telephone and electronic outreach to specific members who may benefit from a more coordinated approach to managing back pain.

Primary duties may include but are not limited to:

  • Serves as a resource to lower-level medical call nurses and physical therapists.

  • Establishes and maintains supportive relationships with members.

  • Performs professional assessment of member’s back pain with accurate and concise documentation.

  • Collects and integrates information in the digital platform.

  • Utilizes established software, written materials, on-line resources and nursing or PT background to provide members with appropriate medical information.

  • Collaborates frequently with multi-specialty team, including external vendors, to assess care management and make recommendations and referrals.

  • Communicates care decisions to patients when needed.

  • Responds to member’s care-related questions via phone or email.

  • May be asked to develop training materials and implement care protocols


Nurse Care Manager I

  • Requires 3 years of case management experience; or any combination of education and experience, which would provide an equivalent background.

  • Current, unrestricted RN license in applicable state(s) required.

  • Multi-state licensure is required if this individual is providing services in multiple states.

  • Home health/discharge planning experience preferred.

  • AS/BS in nursing preferred.

  • Certification as a Case Manager is preferred.

Internet speed requirements: Minimum download speed of 20 megabytes per second/uploads speed of 5 megabytes per second is required.

PT experience a plus.
